Using a Care Manager can save time and money
Our clients may or may not currently have health issues, while some have disabilities, others have chronic or terminal illnesses or may experience an acute event or even hospitalization and still others may be dealing with dementia. When you bring in a Care Coordinator you are not relinquishing your responsibility to your loved one; you are giving yourself the time to focus on really caring about that person.
We work with each patient in the manner best suited for their condition, abilities and preferences. It's important to consider the various aspects of an individual and their family's situation when planning for their care needs: geography, patient wishes, family dynamics, health status, assets and health insurance.
One of the most difficult times for anyone is during a health crisis. Unless you have worked in a hospital or the healthcare field, you probably don't know the questions to ask, what you need to keep an eye on or what you have the right to expect, so calling in a care coordinator to facilitate discussions with hospital staff, advocate for the patient's needs and assist with planning next steps, can make a very challenging situation much more manageable. In order to reduce risk for readmission to the hospital, it's extremely important to make sure all the medications, care and follow-up are secured. Care Coordinators can do this for you.
Listed below are some of the services you can expect from your Care Coordinator.
Services:
- Healthcare and services advocate
- Hospital and facility transition services
- Facilitator and monitor for nursing home services
- Phone and in-person consultations
- Community resource identification
- On-call services
- Liaison, negotiator and coordinator for in-home services
- VoIP services (see KITcam™) for individuals and communities
- Accompany client to hospital and physician appointments
- Other services as contracted

Fees:
Care Administration includes the coordination, advocacy, identification of resources and working with other professionals and is billed at $60/hour.
Direct Care services can include visiting, participating in an activity and accompanying a client to doctor visits, prescription pick-up or errands as needed. These types of services are billed at $40/hour plus travel time.
KITcam™ can be provided for individual clients as part of care administration time with a 2-hour minimum requirement. Special arrangements can be made for communities to purchase blocks of time for residents.
On-Call services will give you the peace of mind knowing there is a professional available to attend to your family member or friend 24/7 whether there is an acute event or a routine need. Caregiving can be exhausting, so it's important for Caregivers to take time for themselves too. ACS provides peace of mind while you are away on fun or business. "On-call" time charges are: $160/3 days, $280/week and $1000/month. The fee will be applied to any time used and will not carry forward to future on-call contracts.
